Families begin with a brief consultation to ensure ClearPath Pediatrics is the right fit. We review your goals, explain our scope clearly, and answer questions before enrollment.

Once enrolled, we complete a structured intake and create a personalized Care Navigation Plan based on provider-issued instructions and your family’s priorities.

This plan helps organize:

• Care instructions
• Appointments
• Medications 
• Communication pathways
   

Based on your selected membership tier, families receive scheduled RN check-ins, secure messaging, and access to education and planning tools.

Our role is to help you stay informed, organized, and prepared — not to make medical decisions.

We help families prepare questions, track updates, and understand next steps so conversations with providers are more productive and less stressful.
